There’s a Fast-Rising Favorite in Facial Rejuvenation, New Data Shows

If there’s one procedure defining facial rejuvenation right now, it’s facial fat grafting. According to new data from the American Society of Plastic Surgeons (ASPS), facial fat grafting boasted the largest increase of any facial procedure in 2025, climbing an impressive 39 percent. Almost half of ASPS surgeons reported that autologous fat grafting is now their second most-used facial rejuvenation technique, trailing only hyaluronic acid fillers at 69 percent.

While the GLP-1 boom has certainly fueled interest in restoring lost facial volume, the numbers tell a bigger story. Data and experts indicate this surge was building momentum before weight-loss medications entered the conversation.

“The philosophy of fat grafting has changed considerably over the past decade. Historically, facial aging was viewed by physicians as primarily a loss-of-volume problem: the classic ‘grape-to-raisin’ analogy,” says New York facial plastic surgeon Matthew White, MD.

Now, with a deeper understanding that facial aging is complex and nuanced, involving changes in bone structure, skin quality, fat compartments, soft-tissue descent and laxity, fat grafting is being used more strategically, says Dr. White. “It is not simply about filling hollow areas; it can help restore balance, improve the quality and vibrancy of the overlying skin and soft tissue and complement surgical lifting procedures.”

Ahead, the surgeons behind this trend break down exactly what’s fueling it, from the GLP-1 connection to why patients in their late 40s and 50s are leading the charge.

Why Is Facial Fat Grafting So Popular Right Now?

While Wayne, NJ facial plastic surgeon Jeffrey Wise, MD notes that facial fat grafting has been a relatively popular procedure in his practice for the past two decades or so, he has noticed a significant increase in demand in recent years. “I think a big part of this surge is people’s desire for a natural filler that is not a foreign substance. Moreover, fat grafting is finally starting to get some recognition for the incredible stem cell properties it possesses. The regenerative potential really is tremendous,” says Dr. Wise.

Dr. White feels the root of the “increase reflects a broader shift toward more durable, biologic approaches to facial rejuvenation.” He notes that part of this change can be attributed to a shift away from other options. “Patients and surgeons have become increasingly aware of the limitations of repeated synthetic filler use, particularly when it comes to long-term maintenance, migration, tissue changes and the potential for an overfilled appearance.”

Austin, TX facial plastic surgeon Yula Indeyeva, MD credits the surge in part to the aesthetic and cultural shift we’ve seen. “I see more people now that wish to look like a more personable, well-rested version of themselves,” she says. “There is a big fear of looking puffy or overfilled. Patients now have more information than ever at their disposal, and can see that fat grafting is a great natural alternative to filler.”

Dr. Indeyeva says there’s also a technical aspect at hand. “The techniques of fat harvesting and processing have refined over the last few years, allowing for more precise placement and predictable outcomes. That gives surgeons the confidence to offer it more often, and gives patients a result worth choosing surgery for.”

Dr. White notes that another piece of the puzzle is “renewed interest in surgical facial rejuvenation, including among somewhat younger patients.” He adds that “Social media has made high-quality facelift outcomes far more visible, and patients now better understand that a modern facelift can create a natural, refreshed result rather than an artificial or ‘done’ look. Fat grafting is often an important part of that equation because it restores volume while facelift surgery addresses tissue descent and laxity.”

The GLP-1 Connection to Facial Fat Grafting

It’s undeniable that rapid weight loss from widespread GLP-1 use has also played a critical role in the increased interest in fat grafting in the face and beyond. “The unfortunate side effect of GLP-1s is that there’s no way to control which part of the body fat is lost from, and the face is rarely, if ever, spared,” says Dr. Wise. “This volume depletion often leads to a gaunt, sunken-in or elongated appearance and fat grafting just so happens to be extremely effective for reconstituting all of this lost volume.”

Dr. White notes that some common consequences of rapid weight loss, whether GLP-1 related or otherwise, include deepening of the nasolabial folds, more prominent marionette lines and hollowing of the temple, under-eye or tear-trough. “For appropriately selected patients, facial fat grafting can be an excellent way to restore balance and soften these changes,” says Dr. White. “However, donor-site selection is particularly important in patients who have undergone major weight loss or may continue to lose weight,” he notes. Dr. Indeyeva notes that GLP-1 patients may have less fat available to harvest, which changes the conversation. “Sometimes we’re combining a smaller-volume fat transfer with other bioregenerative treatments to get the coverage we need. Sometimes, we’re timing the procedure carefully around where someone is in their weight journey.”

Why Patients in Their 40s and 50s Are Turning to Fat Grafting

According to the ASPS report, fat grafting procedures increased significantly, more than 50 percent, in patients ages 46 to 55. The experts say this makes sense as it’s about the time facial aging begins to become more apparent. “This is when patients may notice increasing hollowing, loss of facial definition, skin laxity and descent of the cheeks and lower face,” says Dr. White.

“This age group is experiencing accelerated volumetric and structural changes due to perimenopause and menopause-associated hormonal changes. Estrogen levels drop and lead to decreased collagen and elastin production,” says Dr. Indeyeva. “These are the two main structural and functional proteins in the skin and subcutaneous tissues.”

Additionally, “these patients are also often highly motivated to pursue meaningful but natural-looking rejuvenation. They want to look rested and healthier, not fundamentally different,” says Dr. White. “Fat grafting can be especially appealing because it uses the patient’s own tissue and can be tailored to address age-related volume loss in a subtle, individualized way.”

Dr. White notes that in this age group “fat grafting is frequently combined with facelift surgery, eyelid surgery or skin-quality treatments to create a more comprehensive result.” Dr. Wise agrees, noting that in his practice this age group “often seeks out face lifting procedures, for which we commonly provide fat grafting as an adjunctive therapy. Facelifts provide excellent repositioning of the tissues, whereas fat grafting provides reconstitution of lost volume. When combined, they can produce some stunning results.”
