But they’re not the same. To break down what each approach means, what sets them apart and why the distinction matters, we turned to the experts.

Featured Experts

Sahar Berjis is a registered dietitian and the founder of Inner Health and Wellness

Rekha Kumar, MD is an endocrinologist in New York and the senior medical advisor for Found

What Is GLP-1 Microdosing?

Microdosing refers to administering small amounts of a drug while still gaining benefits. “For GLP-1 medications, that means strategic use to support quieting food noise without making your appetite disappear,” says registered dietician Sahar Berjis, founder of InnerHealth and Wellness. She explains that this often translates to less than the prescribed amount used for weight loss, but just enough to deliver results. “You’ll still enjoy food,” she says. “You just won’t be rummaging through your fridge like it’s texting you every hour.”

It’s important to note that microdosing is a tool used during the GLP-1 journey. New York endocrinologist and senior medical advisor for Found, Rekha Kumar, MD adds that this approach is about prevention and early support as a patient is losing weight.

So what does that look like in terms of dosage? According to Dr. Kumar, microdosing involves clinically guided doses that are lower than the standard amount for weight loss. The idea is to build gradually over several weeks, rather than jumping straight to a full therapeutic dose.

What Is GLP-1 Maintenance?

“GLP-1 maintenance means staying on a stable, clinically effective dose after you’ve reached your weight-loss or metabolic health goals,” says Dr. Kumar. It involves continuing the medication at a steady dose to help maintain results over time. “Think of it like cruise control—you’re not speeding ahead, but you’re also not slamming the brakes,” adds Berjis.

That said, maintenance isn’t always straightforward. Dr. Kumar notes that while some companies market extremely low doses as “maintenance” for people with higher BMIs who have lost significant weight, those doses may not be sufficient to sustain results. “For many people, especially those who started with obesity, the body still needs adequate medication support to regulate the hunger hormones and metabolic signals that contributed to weight gain in the first place,” she says.

GLP-1 Microdosing vs. Maintenance: What’s the Difference?

The fundamental difference between microdosing and maintenance comes down to both goal and dose. “Maintenance is about sustaining the metabolic and weight outcomes you’ve already achieved,” says Dr. Kumar. In terms of dosage, she explains that maintenance requires continued use of a clinically effective therapeutic dose, while microdosing involves lower doses than those typically prescribed for significant weight loss.

Put simply, “microdosing equals flexible and minimal support, while maintenance boils down to stability after losing the weight,” says Berjis. As a result, each approach may be better suited to different phases of the GLP-1 journey. “Microdosing works best for people nearing their goal weight and are looking for light support without heavy appetite shutdown.” Maintenance, she adds, is a more rigorous treatment and is ideal for anyone who experiences intense hunger when stopping the medication.

When GLP-1 Microdosing or Maintenance Makes Sense

Both microdosing and maintenance can play a role in weight management, but for different people and at different stages. GLP-1 maintenance is typically reserved for patients who have already undergone transformative weight loss. According to Dr. Kumar, it’s also ideal for anyone with a history of weight regain after stopping medications, or those managing conditions like type 2 diabetes or insulin resistance.

Microdosing, on the other hand, is designed for a different population. Dr. Kumar notes that it may be appropriate for people with early metabolic risk factors, such as PCOS, prediabetes, a family history of diabetes or hormonal shifts during perimenopause and menopause. “A comprehensive medical evaluation is a standard part of the process to determine if they’re appropriate candidates based on their complete health picture, not just on BMI alone,” she says.

Microdosing is often used earlier in the weight-loss journey. “Those who already have strong, healthy habits in place,” are often the best candidates, adds Berjis.

Weight loss is rarely linear—it’s more of a rollercoaster. Add in individual predispositions, and things get even trickier. When used properly, GLP-1 microdosing and maintenance can help bring clarity to the process. Berjis’ rule of thumb makes the distinction even easier to understand: “Maintenance is like wearing comfy sneakers every day, while microdosing is like only wearing them when you know you’ll be walking a lot.” It all comes down to timing and individual needs.

[55
