One surprising side effect of the medication that rarely gets discussed is vulvar deflation. “Vulvar deflation is a common yet under-recognized consequence of significant weight loss,” explains Louisville, Kentucky plastic surgeon Juan Quintero, MD. As GLP-1 medications continue to transform obesity treatment, both plastic surgeons and gynecologic specialists are seeing increasing numbers of women seeking solutions for changes in the appearance and function of the vulvar region. Ahead, we break down this unwanted side effect and what you can do to prevent or treat the condition.

Featured experts

Sean Simon, MD is a board-certified plastic surgeon in Miami

Juan Quintero, MD is a board-certified plastic surgeon in Louisville, Kentucky

Karen Horton, MD is a board-certified plastic surgeon in San Francisco

What is vulvar deflation?

“Ozempic Vulva” or “Ozempic Vagina,” as it’s colloquially known, first gained traction on social media and is used to describe the physical and aesthetic effects of significant, rapid weight loss on female genitalia. “Vulvar deflation refers to the loss of soft tissue volume, primarily in the labia majora, and to a lesser extent the mons pubis. As fat volume decreases, these structures may appear flatter, looser or more lax, creating a deflated appearance,” explains Dr. Quintero.

How do GLP-1s cause vulvar deflation?

Miami plastic surgeon Sean Simon, MD explains that GLP-1s don’t specifically target genital tissues; rather, rapid and substantial weight loss makes concerns like vulvar deflation more common and increasingly noticeable.

“Significant weight loss affects fat stores throughout the body, including the face, breasts, abdomen, buttocks and genital region. Just as patients may notice ‘Ozempic face’ or volume loss in other areas, the vulva can undergo similar changes,” says Dr. Quintero.

Essentially, what ’s occurring is that “the mons pubis (fatty mound over the pubic bone) and the labia majora (outer lips) naturally contain fatty tissue providing a plump, youthful volume and appearance. As the protective fat pads in the mons and labia majora shrink, the overlying skin (which may also lose elasticity during weight loss) may not contract fully, leading to a deflated, sunken or sagging appearance,” explains Dr. Simon.

Preventing vulvar deflation

While there is no way to selectively prevent fat loss in one specific area of the body, if you’re considering starting a GLP-1, San Francisco plastic surgeon Karen Horton, MD recommends being mindful of the rate of weight loss to minimize the effects of vulvar deflation. “It’s safest to lose weight slowly and gradually; this will facilitate skin contraction to your new form, rather than sudden deflation and hanging/loose skin,” she explains. It also allows the skin’s collagen and elastin to adapt to changing body composition.

Maintaining overall vulvovaginal health throughout a GLP-1 journey is also key in helping to prevent vulvar deflation. “Pelvic floor exercises, such as Kegel exercises, can strengthen the underlying musculature and improve support, although they do not restore lost fat volume,” says Dr. Quintero.

Skin care and hormonal optimization, when appropriate, may also help to maintain tissue quality. “Topical treatments for women experiencing dryness or thinning of delicate tissues secondary to estrogen fluctuations (which may also accompany rapid fat loss), localized vaginal moisturizers or topical estrogen creams can help restore hydration and tissue thickness,” says Dr. Simon. He adds that maintaining optimal nutrition and hydration, including adequate daily protein intake, can help support skin elasticity and overall tissue health as fat diminishes.

Treatment for vulvar deflation

For those experiencing vulvar deflation due to GLP-1s or other forms of rapid weight loss, treatment options vary based on a patient’s anatomy, goals, degree of volume loss and overall health. “The most successful approach is a comprehensive, individualized treatment plan that addresses both tissue quality and volume restoration,” says Dr. Quintero.

For patients who are beginning to notice laxity or volume loss, Dr. Quintero recommends minimally invasive technologies—including vaginal laser treatments, radiofrequency microneedling and other energy-based devices—which may help improve skin quality and collagen production.

Skin and tissue tightening

This involves surgically removing excess skin and using energy-based technologies that stimulate collagen and elastin production. These treatments include:

Fractional vaginal laser treatments

Radiofrequency microneedling (such as Morpheus8)

Internal vaginal rejuvenation devices

Radiofrequency plasma technologies such as Renuvion or FaceTite in select patients

Volume restoration

Restoring lost volume to the vulvar area can be accomplished through the use of these treatments.

Autologous fat grafting (fat transfer)

Donor adipose allograft products, such as Alloclae, for patients who may not have sufficient donor fat

Regenerative adjuncts including platelet-rich plasma (PRP) and platelet-derived growth factor (PDGF) therapies

Biostimulatory fillers

Dr. Quintero notes that in many cases, tissue tightening and volume restoration can be performed alongside procedures such as labiaplasty, vaginal rejuvenation therapies, treatments for mild urinary incontinence, or other functional and aesthetic procedures.

[55
