When hives keep recurring for six weeks or more, they may be chronic. We tapped dermatologists across the country to break down what’s happening in the skin and share tips for managing flare-ups at home.

What ‘Chronic Hives’ Actually Means

The word “chronic” can trip patients up because it describes the recurrence, not the lifespan of each hive, explains Plymouth, MI dermatologist Farhaad Riyaz, MD. “Hives recurring for six weeks or longer are considered chronic, but each individual welt usually disappears within 24 hours. A spot that stays fixed or leaves bruising deserves a trip to see the derm.”

New York dermatologist Dennis Porto, MD echoes that timeline: “Chronic hives are itchy welts that come and go for more than six weeks. Most cases aren’t allergies, but rather dysfunctional mast cells, which are often due to autoimmunity.”

So, What’s Actually Causing Them?

Here’s the frustrating truth: There often isn’t a single, identifiable culprit. “Chronic hives are like a hiccup of the immune system,” says Orange County, CA dermatologist Daniel Chang, MD. “Everyone always asks what the ‘cause’ is, but in most cases there is no one reversible cause—it is often a mistake of a hypersensitive immune system.”

That said, a few everyday medications and habits can intensify flares. “Your headache medicine could be aggravating your hives,” says Dr. Riyaz. “Ibuprofen, aspirin and other NSAIDs can worsen chronic hives in some people. Reviewing occasional OTC medications can uncover an overlooked contributor.” Dr. Porto agrees that NSAIDs like Advil and alcohol may be worth discussing with your doctor if you’ve noticed they worsen your symptoms.

The Daily Antihistamine Routine That Works

Across the board, the dermatologists we spoke to point to one first-line defense: a daily, non-drowsy antihistamine. “The most effective home treatment is a non-drowsy antihistamine like Allegra, taken every day,” says Dr. Porto. “Beyond that, cool compresses and showers, wearing loose clothes, avoiding NSAIDS like Advil, and avoiding alcohol all can help. Cold compresses and showers help through a process call vasoconstriction: The cold signals the blood vessels to constrict, which keeps fluid from seeping into the skin forming the welts.” Fortunately, most people respond well to this approach.

Additionally, Shreveport, LA dermatologist Skylar Souyoul, MD has a tip that chronic hives sufferers may not know: “Many patients don’t realize that they can add Benadryl to their daily antihistamine as needed for hives that arise throughout the day. Topical steroids, such as 1-percent hydrocortisone cream, are also helpful for flares.”

“What you drink with your antihistamine matters, too,” adds Dr. Riyaz. “Take Allegra [fexofenadine] with water: Apple, orange and grapefruit juice can reduce its absorption, making the treatment less effective. It’s a small detail that patients can easily miss.”

When It’s Time for a Stronger Approach

For more stubborn or severe cases, dermatologists and allergists have newer tools in their arsenal. “More stubborn cases are often treated by allergists with treatments such as Rhapsido, Dupixent or Xolair,” says Dr. Chang. These prescription treatments are options for chronic spontaneous hives when antihistamines haven’t provided enough relief; a specialist can determine which, if any, is appropriate.

Tami Buss Cassis, MD, has seen firsthand how transformative these options can be: “Rhapsido has been a game changer for chronic hives. It is important to reach out to a board-certified dermatologist so they can discuss it with you, but my patients with chronic hives are forever thankful for this new drug.”

The bottom line: Chronic hives can feel maddening and mysterious, but between daily antihistamines, a few lifestyle tweaks and, when needed, newer prescription treatments, there’s real relief available. If your hives are sticking around past six weeks, it’s worth booking that derm appointment.

[55
