Is it a filler? How is it different from hyaluronic acid? How many vials do you need? When will you see results? And what about lumps and nodules? Could it complicate a future facelift?

On a recent episode of The Beauty Authority, we sat down with West Palm Beach, FL dermatologist Jacob Beer, MD to get answers. From how Sculptra stimulates collagen to the 5-5-5 rule, here’s everything you need to know before considering treatment.

How Does Sculptra Work?

Dr. Beer describes Sculptra as a treatment that “works in the background.” Unlike hyaluronic acid filler, which can add targeted volume, Sculptra is a biostimulatory injectable that helps stimulate natural collagen production.

“Sculptra will kind of provide gradual volume and gradual improvements in skin quality, tone and texture, but over time,” he says.

That distinction matters when deciding which injectable is right for you. Dr. Beer says HA filler can make more sense when someone needs correction in a specific area, while Sculptra can be particularly useful for more diffuse facial volume loss.

Will I See Results Right Away?

If you love how you look immediately after Sculptra, there’s something important to know: That initial fullness isn’t your final result.

“The thing I tell patients is if you hate it, it’s gonna go away in a day or two. If you love it, then unfortunately it’s gonna go away in a day or two,” says Dr. Beer. The early volume comes from the water used to reconstitute the product. The collagen-stimulating results take longer to appear.

Some patients begin noticing a change within three or four weeks, but Dr. Beer generally tells patients to expect more visible results in about one to three months. Think of it as a slow burn: Weeks or months later, you may simply look refreshed rather than obviously filled.

Do I Need One Vial for Every Decade?

You may have heard the popular rule that you need one vial of Sculptra per decade of life. According to Dr. Beer, it’s not that simple.

“I think that’s a little bit of an oversimplification,” he says. Instead, the amount depends on the individual patient and the degree of volume loss. In his practice, he typically uses one or two vials at a time and may perform two or three treatment sessions before transitioning to maintenance.

Is Sculptra Just for the Face?

On the face, Dr. Beer says he commonly treats areas including the temples, lateral cheeks, preauricular area in front of the ears and portions of the jawline. He generally avoids more dynamic areas around the eyes and mouth.

He also uses Sculptra off-label on the body, particularly the upper arms. For patients dealing with crepey skin there, he may combine it with an energy-based treatment to address skin quality.

What About Lumps and Nodules?

Lumps and nodules are among patients’ biggest concerns, and Dr. Beer says technique matters. Using too much product, improper reconstitution, injecting too superficially or placing it in highly mobile areas can contribute to these issues. “A lot of that is due to technique,” he says.

After treatment, patients may also be instructed to follow the well-known 5-5-5 rule: massage the treated areas five times a day for five minutes at a time for five days. Dr. Beer says the goal is to help evenly distribute the product.

Will It Affect a Future Facelift?

Patients also frequently ask whether previous Sculptra treatments could complicate surgery. Dr. Beer says that, based on his experience and conversations with plastic surgery colleagues, prior treatment does not necessarily rule out a future procedure. Timing, however, is important when considering a future facelift.

“Most really skilled plastic surgeons will be able to work around any sort of filler or Sculptra,” he says, adding that patients considering surgery should discuss the timing of their injectable treatments with their surgeon.

What Is the Biggest Thing to Know Before Getting Sculptra?

For first-timers, Dr. Beer’s advice is simple: manage your expectations. Because the initial post-treatment fullness disappears before the collagen-stimulating effect becomes visible, patients can sometimes think their treatment “went away.” Understanding that timeline beforehand can make the process less confusing.

That slow build is part of Sculptra’s appeal, particularly for patients seeking subtle changes rather than an overnight transformation. It’s also why the treatment often comes up in conversations about facial volume changes after significant weight loss.

Watch the full episode of The Beauty Authority above for the complete conversation.

[55
