The term \u201coff-label\u201d “comes up frequently in conversations about cosmetic treatments, from neurotoxins like Botox Cosmetic to hyaluronic acid fillers like Restylane and Skinvive. It might sound elusive, even a little questionable, but the reality is that it’s extremely common medical jargon for a totally standard, and legal, practice.<\/p>\n
Ahead, we’re diving deeper into the off-label basics, real-world examples and the questions worth asking before you say yes.<\/p>\n<\/div>\n
In the simplest terms, off-label use means a treatment is being used for something other than what the FDA originally approved it for. That could mean a filler approved for one area of the face being injected somewhere else entirely, or a neurotoxin being used for a concern it hasn’t officially been cleared to treat yet.<\/p>\n
If you’ve had Botox (or any brand of neurotoxin) for anything beyond forehead lines or crow’s feet, there’s a good chance you’ve already had an off-label treatment. \u201cSome of the most common off-label uses I perform involve Botox for jawline slimming, treatment of the platysmal neck bands, lip flips, correction of a gummy smile and subtle lifting of the nasal tip,\u201d says Austin dermatologist Kristina Collins, MD. \u201cWith fillers, we frequently use hyaluronic acid products to improve the temples, jawline, chin, hands and even to camouflage certain scars.\u201d<\/p>\n
Not every neurotoxin is approved for the same areas, adds West Palm Beach dermatologist Jacob Beer, MD. \u201cAlmost all of the neuromodulators are FDA approved for the glabellar [elevens],\u201d he says. \u201cHowever, certain neuromodulators such as Daxxify and Dysport are not FDA approved for lateral canthal lines [crow’s feet]. Many practitioners still use those products for treatment of crow’s feet; however, there they are used off-label until they get FDA approval for that specific indication.\u201d<\/p>\n
So how do these off-label treatments earn that trust? According to Dr. Collins, it’s not overnight. \u201cThese treatments became widely accepted because experienced injectors consistently observed positive outcomes in clinical practice, and those observations were supported over time by published studies, expert consensus and advances in injection techniques,\u201d she says. \u201cIn aesthetics, innovation often begins with physicians carefully applying an existing treatment in new ways while prioritizing patient safety.\u201d<\/p>\n
Yes, off-label treatments can eventually become FDA-approved, but \u201conly after the manufacturer chooses to pursue that indication by conducting rigorous clinical trials demonstrating safety and effectiveness for the new treatment area,\u201d Dr. Collins explains. \u201cThe process requires substantial scientific evidence and regulatory review, so many off-label uses remain off-label simply because obtaining an additional indication is expensive and time-consuming, not because the treatment lacks merit.\u201d<\/p>\n
Botox is a prime example. \u201cIt was initially approved for relatively few cosmetic indications but has since gained FDA approval for additional areas such as crow’s feet and forehead lines after strong clinical data supported its use,\u201d Dr. Collins adds.<\/p>\n
Dr. Beer, who runs clinical trials in his own practice, has seen this play out firsthand with injectables that are now household names. \u201cRecently, Skinvive gained FDA approval [for] horizontal neck lines. It initially was FDA approved for skin smoothness of the cheeks in 2023,\u201d he says. \u201cSimilarly, Restylane Contour was initially used for temples off-label until March 2026, when it was FDA-approved for temples.\u201d<\/p>\n
Off-label doesn’t automatically mean risky, but it does mean you should come prepared with the right questions. \u201cPatients should ask how frequently their provider performs that specific off-label procedure, what the evidence is supporting its use and whether the injector has before-and-after photos of patients treated for the same concern,\u201d says Dr. Collins. She also recommends asking why<\/em> a treatment is off-label in the first place: \u201cIs it because the FDA hasn’t reviewed that specific indication yet, or because it’s a newer technique with more limited evidence?\u201d<\/p>\n Beyond that, she suggests asking what product is being used, what alternatives exist, how long results typically last and what the plan is if the outcome isn’t what you hoped for. \u201cThe success of any injectable treatment depends far more on the expertise of the injector than whether the indication itself is technically on- or off-label,\u201d she says.
