The estrogen patch shortage we’ve been hearing about for months continues to leave women searching multiple pharmacies, delaying refills and wondering whether they’ll have to stop hormone replacement therapy altogether. Behind it is a mix of surging demand and a fragile supply chain. Prescriptions for estrogen patches among women ages 50 to 65 rose 86 percent between 2021 and late 2025, and experts say manufacturers haven’t kept pace. <\/p>\n
The good news? Menopause specialists say you likely have more options than you think. Here’s what they want patients to know.<\/p>\n<\/div>\n
If your pharmacy tells you your estrogen patch is unavailable, don’t assume you’ll have to go without treatment. “There are many safe alternatives, and an experienced menopause clinician knows that the patches are not the only choice,” says Chicago OBGYN Sameena Rahman, MD. “There are transdermal gels, mists and rings, as well as oral estradiol, which is safe for most people who need it. There are different types of synthetic pills that can be utilized as well.”<\/p>\n
Princeton, NJ OBGYN Maria Sophocles, MD, says switching to an equivalent estrogen formulation is often possible. \u201cHave your clinician order an equivalent dose of estrogen in a different delivery form,\u201d she says. Dr. Sophocles recommends discussing options such as daily estradiol gels, estrogen mist, Femring, which is FDA approved as systemic menopausal hormone therapy, and oral estrogen. \u201cAny of these can be used short term to control symptoms until supply is more consistent, or long term with the assurance that they are well vetted.\u201d<\/p>\n
\u201cAnother hack is to have your clinician order a patch that is half or double the strength that you need. Some strengths are easier to get. You can cut the patch in half and thereby have twice the amount needed between refills to compensate for delays in delivery or apply two patches to achieve the same dose. Consider switching from twice-a-week to once-a-week formulation patches or vice versa depending on availability.\u201d<\/p>\n
More women are seeking hormone therapy than they were just a few years ago, but both experts say that\u2019s only part of the reason patches remain hard to find. \u201cThe estrogen patch shortage is driven by a combination of increased demand, fragile manufacturing capacity and structural problems in the U.S. drug distribution and insurance system,\u201d says Dr. Sophocles.<\/p>\n
Dr. Sophocles says a cultural shift is also playing a role. “There is absolutely a growing movement in terms of menopause awareness, fueled by influencers and celebrities endorsing HRT, and Gen Xers more willing to advocate for their health than the more accepting Boomers. The removal of the black box warning in November 2025 gave prescribers and patients alike comfort and confidence among those who were reluctant to prescribe or use HRT,\u201d she says. \u201cEstrogen patch prescriptions in women aged 50 to 65 increased by 86 percent between 2021 and the fourth quarter of 2025.\u201d<\/p>\n
But demand is only part of it. \u201cThere has definitely been an increase in awareness of the safety and efficacy of menopausal hormone therapy since the boxed warning removal and due to the menopause movement, however, it is not the reason for the patch shortage, as most would have you believe,\u201d says Dr. Rahman.<\/p>\n
Instead, she points to economics. \u201cEstradiol is not a new drug and there is little financial incentive to make sure patients get this drug. This is more of an issue with who is controlling access to medications, and it comes down to the vertical integration problem in the United States where pharmacy benefit managers process the drug claims and dictate what gets covered and what a pharmacy stocks while being owned by the same insurance companies that patients use.\u201d<\/p>\n
Even with the increase in prescriptions, Dr. Sophocles says overall hormone therapy use remains relatively low. \u201cOverall use of HRT remains very low, around 4 percent of women who could benefit from HRT, relative to the pre-2002 era when it was around 40 percent. It has hovered around 2 percent since 2002 until a recent increase to 4 percent over the last few years. This 2-to-4 percent increase should have been easily absorbed by the health-care system, but because estradiol patches are inexpensive generics with a low profit margin, manufacturers have little incentive to invest in increasing inventory or accelerating production.\u201d<\/p>\n
Stopping estrogen suddenly isn\u2019t considered dangerous in the way some medications can be, but both experts say symptoms often return quickly. \u201cThere is no real risk to an abrupt cessation of estrogen in terms of withdrawal,\u201d says Dr. Sophocles. \u201cBut many of my patients have felt pretty awful right away! This is not a true chemical dependency such as with opioids, but stopping HRT can cause recurrence of insomnia, hot flashes, joint aches, fatigue and other unwanted symptoms.\u201d<\/p>\n
For many women, the biggest concern is a return of symptoms and a decline in quality of life. \u201cThe risk of stopping abruptly is increase in symptomatology, more suffering and poor quality of life,\u201d says Dr. Rahman. \u201cSwitching delivery methods has to be done by someone who knows menopause medicine enough to understand the equivalence between a patch dose and pill etc.\u201d<\/p>\n
Rather than waiting for your preferred patch to come back in stock, both experts recommend talking with your clinician about equivalent options. \u201cThey should call smaller pharmacy, mom and pop pharmacy to see if they have it in stock,\u201d says Dr. Rahman. \u201cThey should ask their doctors if they are candidates for oral formulations or other transdermal formulations or other synthetic patches even. They can try stores like Mark Cuban\u2019s Cost Plus Drugs to see if they have it in stock.\u201d<\/p>\n
If you need to switch formulations, Dr. Sophocles recommends asking about bioequivalent dosing. She also points patients and clinicians to The Estradiol Swap Tool, an online estrogen dose-equivalency resource. \u201cThey and their doctor can use The Estradiol Swap Tool or ask the pharmacist about bioequivalence if switching formats,\u201d she says. \u201cThey should ask their pharmacist what dosages are more readily available, then ask their doctor to rewrite a script which allows appropriate dosing.\u201d<\/p>\n<\/div>\n","protected":false},"excerpt":{"rendered":"
